Help Unlocking sim card

I have an LG G4 H812. I bought it online off someone. It was locked to Rogers when he got it but he unlocked it before I bought it off him. After doing a factory reset recently it seems to have lost the sim unlock code and keeps asking me to input it. I've tried pay for sim unlock sites (about 5) and they all say they can't find an unlock code. I also contacted Rogers and they were more than useless because I don't have an account with them. Looking for another option here. Maybe some sort of way to override the block on it? (the phone is rooted). Any help would be appreciated because obviously without being able to make phone calls or send texts this thing is an overpriced paperweight.

The sim unlock code is provided by whoever your sim is with. Nothing to do with previous carriers. It is a code which locks your sim whichever phone it is in. If you have lost/forgotten it, try 0000 and 1234 and then if no good, ask your carrier for a PUK code for your sim. Last resort is get a replacement sim.

I think (s)he is asking about sim unlocking the phone rather than unlocking a SIM that has become locked, in which case it is the original carrier who would provide it.
 
The phone would not get relocked after a factory reset if it was unlocked by carrier (meaning the person that unlocked it for you did indeed use an unlock code). I don't know much about the G4, nor do I know much about Rogers, but I do know here in the US LG unlock codes are very cheap, if you can not get the unlock code from carrier or any website (I can recommend a site but I don't want to promote any so I won't), then find out how the person you got the phone from removed the sim lock.
