How do you add spaces and punctuation? I never even thought about using it until I came across a thread here talking about dictation causing duplicates.

My approach is to just state the punctuation I want to insert (period). It may not always work (comma), but it generally gets close to what I'm after (period). Maybe that can help you (exclamation mark)!
If not (comma), too bad (sadface)
(period).
