Voter Canvasing App

We need an app to help with voter canvasing.
Hopefully it will have an ability to build a list of addresses, questions to ask, record the answers, and send the data to a base computer, maybe an email address.

Do you know of any such app?
Do you think this will be difficult to develop?

Yes, it is a survey. But I want an app so that I and other can immediately record the results, as the conversation continues, and send the data to a place that will consolidate the results, my home computer, maybe via email. Almost everyone has a phone, the majority of them run Android, therefore, and Android app is needed.
If you have a different position then please advise.
 
Download Link? I don't follow. I am looking for an app that, hopefully, someone has created. I don't have one or know of one at this time.
If I cannot find an existing app, do you have an estimate as to how much time will be required to create one? I can provide more details.

Just keep in mind you are being selective and segregating out those who a) don't have a smartphone, or b) don't have an Android device, or c) don't wish to participate in things like online/app based surveys and voting because of the all the security and privacy vulnerabilities involved.
 
svim: The team doing the canvasing/surveying will have the smart phone or tablet. They will ask the questions, enter the response, and listen to what the residents say. The residents don't need the phone or the app. I will follow the link above and see where it gets me.

Its good for really simple uses. The major downside I found for what I needed this for is I couldnt export the results to excel or sheets where I could process the results into what I needed for my particular application.

If you can't find an appropriate app that will do what you need, this is more of a workaround but you could set up a spreadsheet or database file with the necessary fields you need, and save it to Google Docs. then set that file to be sharable with the necessary people. Your team will use the Google Docs app on their devices when out in the field.
