Help Weird contact problem - invisible contact?

One of my contacts shows up one way in my address book, and another in texting/phoning apps, whether default or otherwise.

The right info is in the address book, but the apps are somehow seeing old information that shouldn't be in my phone - her maiden name and a very old contact title, for example. I've never used either of these on this device. Where is this info coming from, and why aren't the apps using the current contact? I'm only having trouble with this one contact, as far as I know, so I'm at a loss.

Are your contacts synced to Google? Or is this contact linked to her Facebook account (you have Facebook contacts syncing). I experience the same on some contacts, and the info was coming from her Facebook (or Google+, if he/she has that).
 
I've unlinked them and restarted my phone with no change. Also, the email account shown is not attached to anything I had linked. It's quite old.
