Help YouTube App Keeps Failing

Any suggestions as to how to fix this and what might be causing it?

Is it throwing an error or just dropping you back to your home screen. You could always try going to menu>settings>apps>all>youtube ad clearing the cache (and data, if the cache doesn't do it for you). If you clear the app's data, you'll need to re-accept the terms of service.
