help with sd card

I had a DROID x and it broke it also had my SD card with everything on it. I got a DROID RAZR and it says I have to format it to read it.

is there any way to read the SD card or transfer everything so I don't loose it? also I have a SD card reader built into the laptop is there some program that I can download that would read it?

Myself, I just put the card into the laptop and it shows everything without a "reader".
And then I just copy what I want into a folder. Then back into the phone or onto a card in the phone.
