Help Weird ringtone happenings

Has anyone had the phone randomly change their ringtone? I had this problem early on with my DX, but that because of an issue with a Norton Antivirus which I quit using and went to lookout mobile.
 
Are you taking about custom ringtones changing or the default tones that come with the phone? With custom ringtones, I've had this happen with all 3 Android phones I've owned. When it happened on my SIII the other day, I think I finally figured out what happens.
I've always saved custom ringtones and notifications in folders called Ringtones & Notifications on the external SD card. When you remove the SD card or plug the phone into the computer(unmount SD card), those ringtones/notifications temporarily disappear . If the phone tries to access these and they are not there (notification or call ring) the phone "finds" another tone to use, and remembers it. Maybe it notices all are missing, cause it usually messes up all my custom tones.
I moved my custom ringtones/notifications to the internal memory of the SIII (from extSdCard to sdcard). I'm not sure if this will work, but I haven't had the issue since.

It's custom ringtones either MP3 files or sounds from zedge. The odd thing is the sounds for my text, email, and other notifications stay the same, but my main ringtone, and one contact assigned one changed. Another contact assigned one did not (it came from zedge). Seems to happen after a power off event (restart, batt pull, etc).
 
This happened to me frequently on my Droid 2 Global. I finally got tired of it, rooted the phone, and moved the few custom ringtones that I wanted into the system ringtone folder. Problem solved. They even stayed there through a Verizon OTA update. Knock on wood, I have not had to do that with my GS3. I have my custom ringtones on the internal SDcard and they seem to stay put. Also the new USB MTB protocol may be helping.
 
I have this issue with my S3, using the stock ringtones in android 4.0.4.
What I have found out is that it happens whenever I connect to a computer with the usb cable.
When I do this, settings go to the initial ringtone that came with 4.0.4. And the settings read "unknown tone".
Is there a way to get this fixed?
