I still cannot locate exactly what you are using to make this quiz. Typically though there are options to allow for multiple answers to be selected for quiz making programs or websites.

I know that I have used Survey Monkey and it's pretty useful.

Also I think you can create surveys through google as well.

Eclipse is the IDE used to write most Android apps and ADT stands for Android Developer Tools, (the plugin for Eclipse).
