What icon is heart icon?

Samsung Device Care/Digital Wellbeing. Often the notification text would read "x apps were closed due to inactivity' or 'x app was auto-disabled' or 'x unused apps were put to sleep'
This notification icon also shows when you get your weekly screen time report, and is a persistent icon when the device goes to greyscale mode indicating 'time for bed' (Bedtime Mode).

just remember that a heart icon does not mean it is a dating app. it is mostly used for some kind of wellbeing or fitness app.

Do you know what causes the little heart in front of the app name
My guess is that the Digital Wellbeing app is controlling or has limited how much usage you can have with that app. It is a Samsung system app.
i do not see any heart icon on your last image post. can you do a screenshot of the heart icon? what phone do you have?
