there are just too many apps that uses a heart icon for its notification. i would say majority of them are fitness or health apps. the only true way to know what it is is just to ask the person that the device belongs to.
we get this question almost on a daily basis. the problem is that there just way too many apps to know exactly what it is. most of the time these questions get only guesses from us. the only true way to know is to access the phone's notification bar to find out what app it is.
