Help Accessibility Icon (how to get rid of)

I have what appears to be an accessibility icon that appears on every screen even when apps are running.
I'm finding that I don't use it and it always seems to be in the way of text entry or app controls.
When I click on it, it gives me access to volume, power and accessibility settings.
I thought it might be the 'Accessibility Features Shortcut' but changing the on/off switch for that doesn't remove the icon/app

I can't remember how I initiated it and I can't find out how to turn it off in settings.

Any leads?

Hi, I fixed your broken image (hence the post edit). Not sure what the icon is off hand. If it's one of the built in accessibility features of imagine it could be turned off/on somewhere in the settings. On my device, all of the accessibility options are under settings>accessibility

Is there any notification in the status bar for it? If so can you long press the notification and hit the little info button? It should show you which app is causing the notification which might help solve the mystery.

Also, which device are you using? Looks like a Samsung, but which one? I can move your thread to the device section where you might get a quicker answer.

What happens when you long press the icon? Does it bring up any option or can you drag it to the trash?

Thanks for the quick response Brian.
The phone is a LG G4. It's with Telus.
When I long press .... I get nothing. No options open up to remove or move.

When I short press I get a window with Volume up, Volume down, Power, Home (and a next button)
The Next button gets me Screen Capture, Pinch, Accessibility and Voice Mate (and a previous button)

The app icon itself is the same one used in 5.1 for Accessibility in Settings.

I would guess that that is where the option is ... but I just can't find it!

I'm going to try to attach a couple of screenshots .... hope it helps to explain my issue. These are the options when I click on the icon. I have attached the icon as my avatar. Anyway, anyone?

Go to accessibility than click on motor and cognition and to turn off the accessibility icon turn the button to off
