Adding a contact while conversation

Are you using a dialler application to make calls?
The scenario is as follow:
brand new Nexus 4, No ROM or other installed on(bought on Google store, no changes have been made).
1. dialing or receiving a call.
2. clicking the add contact
3. the screen with numbers appears.
4. the other side can't ear me unless I'm moving back to the "current call screen".

Or maybe you're face is just too far away from the mic? Or are you on a headset?
I will try it, but as I understand it's just a workaround to my problem, to put the phone or notes somewhere else.
Do you know if the problem should be solved by new version of Android or by a new version of ROM?

No at all, I have tried it all! It doesn't work. the mic is muted, when I go back to the "current call screen" the mic is back on.

I got the answer on another forum, I want to post it here too to help others!
During a call I wanted to add a new contact, I pressed the Icon of the add contact, apparently it's the Icon of adding a contact to the current call(conference call), the proper way to add a new contact to the contact list is to "go down" to "desktop" and from there adding a new contact.
The same Icon for adding someone to the conference call or adding a new contact is confusing.
