Alarm problems

It's happened a few times since the ICS update. I use Alarm Extreme and sometimes the alarm will go off, but no sound will play. I have a SA set up for night time which will turn the phone to silent but usually the alarm will work like it should and play anyway. Sometimes it won't. Anyone else having this problem?
 
When you upgraded to ICS, did you do a Factory Reset after or just let the upgrade automatically handle it?

I use the stock alarm, linked to a song, and it goes off like clockwork. Yes, I wake up to "big ol' jet airliner".
