Android Not Connecting with Computer

Every Pan drive is connect to my Computer USB but only problem with my Android cell.
I have chance my usb code still the problem stays...

1. Your exact make/model device and carrier
2. Your exact problem
3. Any extra symptoms that would help us diagnose

Where there any changes to your phone right before the problem started?
