Virtual Network Computing (VNC) was developed for that need.

Install and configure a VNC server (there are several free ones available) on the PC, and a VNC client or "viewer" (there are several free ones available) on your phone.

Make the connection from the phone, and the PC desktop is right there for you to control, if you configured the server to allow it.

If you're just looking to control the PC via a remote device (for instance it is connected to a TV) I like Unified Remote. If you want something to view and control your computer on your phone then I recommend Teamviewer..

A few months ago Amazon had a free app of the day called "Jump".
It's normally $10 if memory serves. There's a free server you download from the developer's website.
I tried it and it works great. I can control my PC from my tablet (Samsung Galaxy Tab 10.1) perfectly.

There are too many to name - I use Imperio by the way....

Incidently for most of the remote desktop apps your Windows version needs to be capable of hosting a remote desktop session. Basically that means it has to be Vista /Win 7 /Win 8 PRO version or higher. Look for the Windows settings in Control Panel, System, then on left pane click Remote Settings, then you should see something for settings of "Remote Desktop". If it blank you can't host it and our probably running Windows Home edition or something like that.
