Automatic Forwarding When Home

I have a GS3 with Verizon and have horrible service at home. It's not just a Verizon problem, I had the same issue with AT&T. I just live in a dead area.

I would like to find an app that will automatically forward calls to my home phone whenever my GS3 is connected to my home WiFi. That way when I'm at the house people who call my cell phone will actually be calling my land line. When the GS3 disconnect from my home WiFi, calls would come to it normally.

Is there an App that can do this?

I would have thought this would be a more generic Android question. It's not particular to the phone, it's a question about an App that would automatically turn on forwarding when the phone is connected to a particular WiFi network.
