Bad SD card? Bad reader in phone?

So I have been getting errors lately about my SD card not being safely removed...when I haven't even bothered with it or anything. Then last night I go to my gallery and nothing, SD card not mounted or present. I reboot and see a message about a damaged SD card...so today I reboot the phone and plug it into my PC and I can explore the card, so I backup the entire card to my desktop. Now I can't even get it to show up on my PC either, I get a "Device not recognized" error...even looked under Disk Management to see if it at least saw it, and needed to format it...

So question is, does it need a format, or does it sound more like it's damaged beyond a format? Or could it be the reader in the phone? I don't have another reader to test it in...

How would I format it if I can't even access it?

I had problems with my sd card - the phone kept saying that it was removed. I put all of the sd card contents on my computer, formatted the card, and the replaced the files. All is well since then.

I was able to get the PC to read the card, did a backup, formatted, and so far so good...

I had a similar issue. Upon reboot my phone said my card was an invalid format. I shut down and took out the card and put it into my PC, which read it fine. Backed up the card and reinserted into the phone and it worked fine, no format needed. It's the second time this happened, first time I clicked format before I knew what I was doing, and oops, all my data was gone! At least I thought it through this time.
