What could possibly cause this?

There are some prior events that might be part of the reason, although I can't be sure:
For a long time I had a steady battery life of about 5 days and 20 hours. At some point, possibly after an OS update, the battery life suddenly dropped to a steady 4 days and 20 hours. This was not a gradual drop, and instead happened pretty much overnight. I do not know if this was because of an OS update, or perhaps a sign of a failing battery. Also, I have no idea if this is related to the above issue.
However, recent events are much crazier than that. I recently forgot the phone on the charger even though it was on full charge, for something like 10 hours. Of course normally this shouldn't be any kind of problem, as I assume phones are designed to not break from that. I disconnected the charger from the wall socket without waking up the phone or disconnecting it from the charger. Again, this shouldn't be a problem either.
However, the phone started acting really weird. The charge percentage was stuck at 100% for like a day, and the phone would at random times issue the signal of "battery full, disconnect charger". I shut the phone down and restarted it, and now it showed a battery percentage of about 70 or such.
Some days later, when the battery was pretty much drained, I recharged it, and a couple of days later I got what the image above depicts, which has never happened before.
As said, I don't know if any of this is related to the problem, but wanted to be as specific as possible.
