Help Camera Counter Reset Causes

I would like to learn all possible causes of a camera counter reset (and, if possible, their relative frequencies). From browsing forums, I have come across the following possible causes: (1) factory reset, (2) flashing a new ROM, (3) installing an update, (4) manually changing.
