my camera icon has vanished and cannot access my phone

If you mean it's vanished from a home screen, just put it back: on most launchers you would do this by long-press on the screen where you want it, then select that you wish to add an app rather than a widget, then select the camera as the app.
If you cannot see it in the app drawer or the home screen, check it is still on the phone by going to Settings > Apps > All, then see if it's there. If so, check that you haven't disabled it by accident (which would make it disappear).
Not sure what you mean by "cannot access my phone" though. Do you mean that you are locked-out of the phone, or that the phone app has disappeared too? I know you can't answer, so please post again with more information and a clearer description of the problem if the link here doesn't give you your answer.
