Can I root an HTC Thunderbolt with ICS?

You mean, your phone is on ICS nd you want to root it, or you want to root and upgrade to ICS?

Hi and welcome to the forums. In short,yes you can root the thunderbolt even if it has ICS. But it isnt as simple as that. It involves actually downgrading to gingerbread to get the root. I highly suggest joining the forums and speak with other owners of the thunderbolt who can guide you through the process.
