Help Can't turn off call rejection

My Galaxy Note 4 insists on rejecting calls from unknown numbers - even with call rejection turned off (or turned on and Unknown numbers unticked). This is a problem! It feels like something other than the Phone app is doing the rejection, but I've no idea what - there are no obvious candidates.

I've had Lookout installed, I've just downloaded Avast and run a scan as well, nothing.

Any suggestions?

If it's a GSM phone try the Sim in a different phone. If it still does the same, call your carrier. If it works ok, I don't know!
