Change MEID for Boost Mobile

Hi, I have a US Cellular Galaxy s2 and I need to change the MEID to be used with boost mobile. could anyone help me out with this or point me in the right direction?
 
I actually have that but have no idea how to use it to change my us cellular MEID to a Boost mobile one. I've been looking online for hours trying to figure it out before coming here.

not sure how to use it as well try youtube to look up videos on how to use it.
