Chekbox list app?

[killmister]

I am searching an app for creating, editing and viewing lists with multiple checkboxes. I tried Documents To Go, Quick Office Pro, Google Docs, but none of these can do this. I tried Note Everything but there is no multiple checkbox I post some example of form I try to make on my cellphone:

I just tried it. "Kind of" works. Not a lot of formatting options that I could find (like centering, column resize). Can't make a checkmark, but can make an "x". Easiest to make it on PC in a spreadsheet, then just read it on the phone (I think several apps will read spreadsheets).

I found an app - ListWizard Pro version, that has Boolean fields, with import and export Works fine...
