My Clock and Weather icon(s) disappeared off of my phone. How do I get them back?

Hello. Try long pressing the area of your screen where the clock/weather widget was. A menu should appear with widget as an option. Choosing widget you will next see a list of widgets that you can place in that area. Find your clock/weather widget and choose it. It should be as easy as that. I hope this helps.
