Does anyone know what app sends this notification?

It looks like it might be from a health or fitness app to me. It's very rare that these questions get answered because there are millions of apps out there and unless someone specifically uses the app, we simply don't know.

The best way to be sure about it is to ask the person who sent you the screenshot.

If you pull down the notification shade on your device, it should indicate what app it is.
But invariably the icon is from a screenshot from someone else's phone (who the poster usually doesn't want to ask).

Here I agree with Brian: the "progress bar" thing around the heart looks like a fitness app to me rather than what I suspect Laura is concerned about. But it's not one that I recognise myself, so I can't be certain.
 
Of course I didn't recognise a "Digital Wellbeing" notification because I've turned-off notifications from that thing. But it irritates me that Google won't even let me disable it: it's of no interest to me, disabling notifications means that it has no function on my phone, but it still has to run because Big Brother Google says so. Nothing on the phone depends on it, so it's in no way essential, so why can't they let us disable it?
