Okay, so I think this is a software issue

If i hold my phone up to my ear during a call no one can hear me
If its horizontal and I speak right into the mic they hear me fine

However OUTSIDE of a call the mic works fine
Google voice search thing can hear me perfectly fine in both positions
and a voice recorder did the same.

So its only happening during a call, I turned off voice privacy already and that did nothing.

Here are a few suggestions/thoughts:

Do you have a case? its it perhaps blocking the mic port when you are holding it a certain way?

Placement of the phone to your face also plays a very important role in your mic sensitivity

Generally there are no software issues that would affect the performance of the mic within calls, unless you are rooted and used custom software that made a change to the mic gain.
