Do you have anything like a screen protector or hair obstructing the proximity sensor. Two scenarios I have seen:
I had a screen protector partially covering the proximity sensor on my GNex
My wife was holding her OG Droid on top of her hair instead of against her head under her hair (later fixed with an OTA)
In both cases, the obstruction of the proximity sensor was causing the screen to stay on while talking on the phone and causing face hangups when your cheek came in contact with the area of the screen where the end call button is.
