IN the UK we have 'Working at height' regs- anything over 3 Metres/10 feet you have to use a scaffold tower, Ladders under that height are OK.This isn't a failure.. not yet anyway. Every OSHA agent that views this meme will surely have a heart attack.
The US has strict and enforced regulations for safety. There is an entire bureaucracy in place to police the unsafe. Their eyes can't be everywhere and rules are often ignored.
Which is a good thing because we enjoy a lot of "hold my beer" moments...
