Has my device been hacked?!

I have a 2nd generation Moto E on 5.1 lollipop. It is the Verizon LTE version and it is acting crazy.

I reset the phone a day ago because I had the headphones in and Google search kept randomly interrupting my music and the volume was changing by itself. * Let me also add the my headphone jack is kind of messed up and rotating the cable or moving it will sometimes interrupt ground to the speakers. Ive had several cable seem to fail in the L channel ). I know most phones have a pin for inline controls on TRRS port, so perhaps this is main problem?
After I reset the phone the ghost button touching seems to have mostly disappeared but now it's doing other things like the screen lock disabled itself or ' failed to work ' one time.
I'm sort of worried that it's infected or compromised because it acts very erraticly and has persistent misbavior that continues after factory wipe.
The volume control has always seemed to have a mind of its own. It often will turn itself down and it will bring up the message about high audio volume and I tap OK...

Is this a hardware problem or is the operating system corrupted/ hacked?
What can I do to test this??
 
To be honest I think it far more likely it's a hardware problem than that the phone has been hacked (hacked in a way that makes it obvious that there is something wrong while serving no obvious purpose at that). Most of the problems seem to be related to the volume control, one way or another.

Has the phone ever got wet? Moisture damage can do all sorts of odd things, and it can carry on developing over time so appearing OK initially does not necessarily mean you got away with it.
 
I have to point out that the phone does not necessarily have been exposed to 'water' per se to get water damage. Steam from showers and rain can ingress into the phone and cause water damage. With people often bringin their phones to showers nowadays, it's a common issue for non water resistant devices.

Yes, as a matter of fact it's been a bit wet in areas that might be an issue, I've used wet paper towels and such to clean the screen because it's so attractive to grease. The speaker area and around the headphone jack might have gotten wet

Lol. Yeah, I've taken in the shower.
Also its been in my pocket at times when it was pouring rain. Never directly exposed but it's been quite damp at times

A good sturdy case helps with the rubber sealing some of the seams, but it still leaves vulnerable points (headphone jacks, ports, mics). So it's possible. If it can't be fixed I suggest going for a water resist unit.
