Ok so I need a notepad App! BUT... not just your ordinary notepad app!

I am in Law Enforcement and am desperately trying to find some sort of app out there that has editable fields in it...

IE:
A Field that I could rename FIRST NAME and then always be able to enter text after it

Preferably with a Comments section under neath it.

Alternatively...

I am also looking for a notepad that I can add subnotes to it, kinda like bullet points except they are completely separate notes that pertain in some way to the parent note.

Does anything like this exist??? It would help make my life SO much easier...

Have you tried "police pad"?
https://play.google.com/store/apps/details?id=police.pad.app&hl=en

Just tried it and nope.

I need something that has the ability to allow me to add my own descriptive fields seeing as I am putting down the same information all the time. I hate writing out Name, DOB:, SID#: and all those descriptors...

Looking for more of a Form that I can edit the fields and then just fill in the blank. Also would be nice if I could e-mail them too

I believe I have what you are looking for. I will describe it as best I can and then you can let me know if it is what you are looking for. I have made my own Print order form for my business which has:

Job Title: this field is editable
Qty: this field is editable
Size: This field is editable but in addition I have made this field a drop down menu from which to choose predefined sizes.

These are a brief descriptions of fields I have made, you can customize your own. In addition I have a "Notes" where I can add any pertinent info. You can even add photos to the document if you want to. I created these fillable form fields in Adobe Acrobate as a pdf form which I keep on my phone then just fill in the info.
You would need an app called "ezPDFreader" in the market.

**I can make the form for you then you would just need the app I described above.
***If you are willing to buy the app for a couple of bucks, I could send you my form for you to try out. I am sure it is what you are looking for.

That is pretty much it! I'd need at least 9 fields + a large Description Box at the bottom for Additional Information.
