How to know if an app is running in the background

There is a certain game I was playing and a couple times I didn't close it (I just went to home) and then a window popped up later and said it has been running for 45 minutes and eating the battery and asked if I want to close it. It also sends notifications. That made sense to me that it was running in the background and the battery was draining fast.

If I actually exit the game, and I go through the process and am pretty sure it is exiting because it asks "are you sure you want to exit?", I can go in the app manager and it does not show it is running, however I still get the notifications from it. Is that possible, or can it still be running even if it isn't listed under running apps?
 
If you go to setting --> Manage App --> Running...you can see what apps are running. I would assume that this app has been. When in running, you can stop any app that is still running.

Edit, I see you already did that. Try what Bodycount said, if that doesn't help, uninstall the app and reinstall it. If it keeps happening, I think you have a bad app on your phone.
