Root [Verizon] How to re-lock phone, and get rid of "Custom" on boot?

At some point in the past I think I tried to unlock my phone (Samsung S4 on Verizon)). Now when I boot I see the "Custom" with an open padlock icon.

I am trying to use my phone to access my work email. The company has a system that checks the posture of the mobile device and will not let it connect if the OS appears to have been tampered with, which I guess mine does.

Is there any way to get it back to its out of box state? I've tried doing the factory reset but all that does is delete my data and default settings. I still see the custom icon and the posture check still fails.
