I lost 2 homescreen panels on reboot

I made 7 homescreens for my Note 4, and after I rebooted (well, I let the battery run out and it forced reboot) 2 of them were gone. One of them had my music widgets on it, so I know it wasn't an empty page. I can't remember what the other one had.

Any ideas?

Are you using the default Touch Wiz launcher?
Any other changes to the phone from default?

Yes, stock TW Launcher. No other changes that I can think of. I tend to like what Samsung puts together, and usually keep things stock. I only root if there's a block on an important feature that I must have. I've rebooted twice since then, and I still have 7 screens, so maybe it was a 1-time hiccup.
 
I've just had another problem that I think is related to this one. I'm calling it a homescreen-persistence problem. I move two widgets from homescreen 6 to homescreen 5 (Pandora and Play Music). The widgets themselves moved without issue and were no longer visible on page 6.

I rebooted my phone and the widgets are now on both homescreens. I also just remembered, I put the 4x3 calendar widget in place of those two music widgets on page 6. The calendar widget is gone, and as stated, I have a double occurrence of the music widgets.

Another issue that may not be the same underlying issue (homescreen persistence) deals with the S-Note app (widget). As you make action memos, they are saved on the widget and stacked on on top of the other. Yesterday, I had deleted all my action memos as I had finished with them all. A few minutes later, I made another one, and the action memo that appeared in the widget was one I had deleted. When I clicked on it, it did lead to the correct memo, but the displayed memo (icon) was incorrect.
 
I have no other suggestions except to do a factory reset and try those things you are experiencing before putting other apps on. Then start putting a few apps and keep trying those thing so you may determine if an app caused this.
Your phone's behavior is odd.

Try SAFE MODE 1st before going to the nuclear option:

The method given for the NOTE2 also works for the NOTE4.

While in SAFE MODE,one by one enable your user-installed apps until the problem presents itself,then,you've likely indentified the culprit.
Uninstall the suspected app,reboot your phone & monitor for results.
