Incoming Phone Numbers spaced out?

Hello you guys, ill make this quick. So i have a note 5 that was on verizon but now on At&t. Connection is good no issues.
My only issue that i just noticed earlier today, which i'm sure wasn't happening yesterday is that. When i get a incoming call, the Third Number is always spaced apart (Third Number being part of the zip code usually)
Even if i have it as a contact, it will not come up as if i have it in the address book unless i add it to it with the space. Its freaking annoying because i would get a call from a friend and it will pop up as if i don't have the number saved. Text messaging is fine though.
Someone please help me, i need my phone for my business lol
Attached is a screen shot of an example.
The "Office contact" Wasn't showing up as saved until i saved it, but look at it.
And the other one, isn't saved but a familiar number.
Please help me out ):

For best exposure to other users of the app this thread was moved to the forum dedicated to discussing your phone.

(As an example I use Pixel Phone Pro on a Motorola Droid Turbo 2 and it is totally different from what you are using.)

I've never seen this before & apparently I have plenty of company,judging from the lack of responses.
Have you tried contacting your service provider &/or Samsung regarding this issue?

And no i haven't. I just got the phone a few days ago, and my contract with atnt was up already. So i just swapped the sim card into the note, i previously had a iphone 5s
Would that have effected it?
Should i contact atnt or samsung lol

I'd start with AT&T,being the easiest/quickest starting point & possibly getting a new SIM Card.
 
And i just reset the phone, and still the same thing :(

But okay i will!
Hopefully something can be done about it
I was gonna suggest a reset,as the reps @ AT&T would likely recommend doing so before they give you a new SIM Card (hopefully they'll replace it w/o charge).
Please keep us posted on your results & BTW................

Okay guys, so i went into Atnt yesterday and they said they havent seen this before, and they wouldnt even offer to give me a new sim card ( I went to two locations and they said the same thing)
And then i went to verizon and they said they have nothing to do with it.

So i just fixed the issue of when i would call back on these numbers that are spaced out, it would dial 011 infront of the numbers. I guess it had to do something with international calling, i tried to fix it through the phone and no luck so i downloaded the Prefixer app, and it worked!
But im still receiving the numbers spaced out at the 3rd Digit!
Ugh it sucks! ):
