Root issue getting sd-ext mount consistently

I am running into a consistent issue getting my sd-ext partition to mount.

Ive tried both formatting through cwm 5.0.8 and through a windows based utility with ext2, ext3, and ext4.

Ive flashed my rom a dozen times at least with an attempt to run an a2sd or similiar script right after, and only on a couple occasions has it worked ( rest failing to no mounted sd-ext partition).

Im pretty sure when I had dt's a2sd running solid and ran into out of room errors, it was because the sd-ext partition stopped mounting.

What is the cause of this and how can I work around it?

What has always worked for me is to back up your SD card, use CWM to partition the card, make sure you choose 0mb swap, then reboot and use Link2SD to make the second partition as Ext4. It will want you to reboot before doing anything and you should be good to go. I have never had any issues with this process other than when you remove the SD card while the phone is on, lol
