Help Issues with New GSM Galaxy Nexus Stuck on 4.0.1. Please Help

I just purchased the Unlocked version from Amazon to use with ATT and I really like it. However, I'm having a couple of issues with the phone...

1) My phone is stuck on 4.0.1 and according to the system update page, I'm up to date. Any advice on how to upgrade?

2) My phone will randomly turn on airplane mode. When I turn it off, there i2 still no signal and I'm forced to restart my phone. Any suggestions?

3) When my battery gets down to 9% or 10%, the screen will not stay on. I have to press the power button multiple times to get the lock screen on and when I unlock it the display closes shortly after that.

4) Any suggestions on extended batteries? I got the 3850mH QCell. I mistakenly didn't let it go through for a full charge. Perhaps as a result, or maybe not, this extended battery only lasts me ~12 hours on moderate use. Did I do irreperable damage to the battery? Is there a better one available?

Thanks, I will try to do that as soon as I'm near wifi so my apps are restored. Any advice on the update item I just added to the OP? I didn't even realize I was still on 4.0.1 but it seems like I'm not alone on this issue...
 
After a reset, most of the problems still persist and I still haven't been able to update, even though I checked that my GN was the yakju version. Do I have a defective phone?
