It can be modded one (different ring) with nice clock widget

Changing ring with xposed's gravitybix or some other modules is easy

Not sure but you could try going into settings/apps and choose running and see if you can find the name of it that way. May have to do it by a process of elimination If you are unsure of an app name and you think it could be it Google it and see. Not the most preferred method I know but it could work.

Possibly Widgetlocker being used. The clock looks like a Zooper Widget I've seen before (unfortunately I can't find the link).
