If you have os 4.1.x you can use CDMA workshop. I am guessing you flashed a custom rom? If that is the case revert to stock odin. Use cwm/philz_touch or twrp and Backup your efs partition which you can restore AFTER custom rom. My other guess is you were messing around with CDMA Workshop or the service/programming menus? If it were a esn related application you should be able to revert your changes from the BackUp. This is typically choose Backup file and writing it to phone.
I would be extremely careful flashing/writing a strangers QCN File. A QCN file is the only permanent software modification that I know of for a cellular device. Should that file be corrupted/modified accidently or otherwise you will have a brick or permanent cellular connectivity loss.
Everyone always says to backup and this is especially true. If you did not then do it now before anymore problems occur!
I have a D2MTR (True MetroPCS). I can dig around and find my qcn file, but do not suggest you use it.
TLDR: Eastiest method is to use EFS Pro or CDMA workshop and write the esn listed under battery back to phones nvram.
