Help Lost an applications icon

Hi all, odd one here on the Galaxy Note 4.
I appear to have lost the icon from my 'phone application which I kept in the quick access bar at the bottom of the screen. The application is still active and launches but now it just says a rather ungrammatical, "phone" with no picture.
I have tried deleting it and going into my main applications reservoir but even in there it is just present as a word, with a blank where the icon ought to be.

I stress I haven't lost the application or its functionality, just its icon.

If I go into Application Manager, aelect All and scroll down to "Phone", the icon is there (3×3 green mosaic with white telephone).

Might I have deleted or dissociated the graphic by mistake? Are they kept somewhere else in the memory and are they restorable?

Very strange!

Unless you're rooted, it's unlikely you've deleted it.

Have you just taken a system update or anything like that? Are you using an alternative launcher with another icon pack?

I'll move this to the Note 4 area, see if they have any ideas. :)

remove the icon from the launcher bar. go to settings> applications> application manager> scroll to ALL> select Phone, and then Force Stop and Clear Data. If you can, Clear Defaults. Restart your phone and try again.

It's also possible there's a software burp on the phone and you'll have to factory reset. Post up if the above steps worked or didn't.
