Help Lost Phone Icon - and it not in App tray

(When another person asked this question, they found the Phone icon in the App tray)

This is a standard ATT Galaxy S5 - standard launcher

A child was playing with my phone and the Phone icon is gone from the home screen. There is no Phone icon in the app tray under any name (Call, Dial, Phone). There is no icon in the Application Manager either.
 
DO you have a tool box app? I had the same problem and it kept ending up in my toolbox. Check there and in any other folders you have on your desktop.

The Phone is not in the Tool Box. I stopped at an ATT service center and they just said its GONE and that I should reset the phone to default settings.
 
on my Verizon S5, if I pull up Contacts, I can get to all the Phone functions; don't know if that'll work for your device.

Contacts are also gone from the Application Tray. Since phone and contacts share an interface, maybe they disappear together.

I will probably just reset the phone to factory defaults.

if you go into the app tray and tap the menu ••• (vertical, of course), is show hidden apps greyed out? if not, see if that is what happened. you could also take a look at disabled apps, same menu.
EDIT: on my phone, I cannot disable Phone or Contacts, but they can be hidden....
