Medication Tracking Application

I'm looking for an app that will track what medications I'm on - OTC and prescription. This must be easy to get to when I'm in the doctor's office and be accessed by emergency personnel in case of emergency. I would also like to track each member of my family's medications. I don't need reminders to take my medications, but wouldn't turn down an app that has that feature. Any recommendations?
 
Don't know any medication-specific apps that list your meds, but any app that allows you to make lists could work.
 
Drugs.com has a handy mobile app for traking your medications and informs you about side effects and interactions between the medicines you take. Try it out, it is called personal medications record.
 
Keep Track and Keep Track Pro are two apps that you can use to list and track your medication and usage of it, if you don't like the Drugs.com one. You would put in the information yourself and set it up how you want it.

You can also use it for a variety of other things.
