Notification sounds changing on their own after last OTA update

A few days after the last OTA update a few weeks ago, I noticed that I wasn't getting any notification sounds on my phone. So I went into each app (Gmail, email, texting, Hangouts, etc) and they all have been changed. All of the sounds were assigned a name/number (like "9154" or some random numbers) which seems to not make any sound at all. As soon as I changed all the sounds back to what I wanted, they worked fine...for about a week until they all changed back to the silent number-named ones on their own again. This has happened 2-3 times so far.

I have no actual proof linking this issue to the last OTA update, but this never happened before I did the update.

Any ideas?

No clue as to why it keeps resetting, but after the 4.3 OTA last week I discovered that all ringtones for entries in my address book had reverted to "default." However, they have stayed changed once I reset the ones that were other than default.

also make sure ringtones are on the phone not the card
