My S4 is rooted and unlocked. I'm having trouble upgrading to 5.0. How can this be done?

A rooted phone cannot receive an OTA upgrade. Your best bet is to download the firmware for your model of phone, CSC and Product codes and flash it as per the instructions and video contained in the links of #1.5 of 29 Galaxy S4 models - know yours? Flashing the stock Samsung firmware will, of course, unroot you and you will need to re-root again.

If I unroot the phone, and return it to oem what will Lose, ie data, apps.

Not usually but as with any procedure of this nature, one should always back all data up beforehand... just in case.

Would I use the factory reset button to unroot? This button states data would be erased

Unroot using the Settings in SuperSU app.

You can try unrooting, but if any system data has been modified the OTA still won't work. You're best off just backing everything up, reflashing the firmware with Odin, and starting over.

Hmm I'm not familiar with the cricket version but you can go to SamMobile and download the firmware for your device. You'll need the exact model number. Then follow the instructions and use Odin to flash the latest firmware.

There has been no official update to Android 5.0, Lollipop, for the carrier branded minority variant for the U.S. carrier, Cricket, on model number, SCH-R970C. See, here. Cricket seem to have abandoned this phone. Latest release is on KitKat, Android 4.4.2.
