Help Phone calls stall phone

I need help. When I place a call the other phone rings but it appears on my phone as nothing happened. The button does not appear to be pushed and the active phone call screen does not come up. When I get a call, the phone call appears but I cannot swipe to accept or reject. I have already reset my phone to factory and restored all my apps, contacts, etc. Any ideas?

Sorry to hear about the issues.
Try running in Safe Mode & see if the problem persists.

If it goes away,then we know one of your downloaded apps is the culprit,finding which app is a simple process of elimination,start using installed apps one-by-one until the problem arises once again.
To escape Safe Mode,simply re-boot your device.

Thank you for the suggestion. I attempted making a call in safe mode and there was no improvement. My outbound call froze my phone and I could not hear the person answer it and they could not hear me. When she called me I could not answer either.
 
OK,well that leaves a couple of options:
1) Visit a VZW store & try a new SIMCard.
I'm not certain what,if any,effect the SIM Card has w/calls on VZW,but,on GSM phones,you can't make a call w/o it. Wouldn't hurt to try.
2) If #1 fails,while you're there,see if they can re-install the firmware O/S.
