phone icon in folder

Hi - My Galaxy Note 2 has somehow placed my phone icon in a folder. Now every time I want to make a call I press the icon and it opens up the folder so I can click on it again to make the call. Don't know how this happened or how to remove it. It looks like the icon is hovering over a hole.
 
Press the folder to open it. Long-press the phone icon and drag it out of the folder. Then put it where you want it.
