Phone suddenly PUK locked?

All right guys, I've got a bit of a problem here.
I'm running a Rooted Insipre with 4.0.3. I don't know anything about rooting and my friend did this for me, so all I know is it's most likely using ClockWorkMod if I'm looking at this right.
The problem is: I enabled the SIM card lock, so whenever I turned on my phone the first thing it would do after boot is ask for a PIN to unlock my password. About maybe a month later, today, I woke up and booted my phone to find now it was asking for an 8-digit PUK code along with a 'New Pin Code'.
I have no idea what a PUK code even is and I never set it.
I've gotten to the point of having factory reset my phone and it's still asking for both codes.

Call AT&T, dont try to guess because you have limited guesses.

Edit:
A PUK (PIN Unlock Key) code is required to unlock SIM cards that have become locked following three successive incorrect PIN entries. If your SIM card becomes locked, you will need to get a PUK code from AT&T to unlock your SIM card.

If you enter the wrong PUK code 10 times in a row, your SIM card will be invalidated, and you will need to purchase a new SIM Card. The phone will display the following error: "PUK blocked call operator."

Here is the PUK Code that you need. 13937055. It will then ask you for a new code, that has to be between 4 to 8 numbers. So use a set of numbers that you will remember.:)
 
Your SIM card can be 'voided' if you use the wrong code 10 times in a row...and then you will be forced to purchase another? Wow. I had no idea.
