I personally don't have this problem with my N5, but have read that a few others are experiencing the same problem as you.
I guess Google is aware if the problem and might release a patch/update at some point to fix it as it seems to be a software issue but there's no ETA on the fix.
The problem seems to be with the Noise Cancelation Microphone.... Which is located to the right of the headphone jack, that little port hole there on the top of the bezel. It seems to be a problem more with the 32g version. Is that what you have?
Something's you could try to temporarily remedy the problem -
To start and to verify if the NC microphone is your problem, try covering the NC microphone port with your finger and make a call and see if the other caller can hear you any better.
If so remove you cellphone case (if you have one) then you can cover the port with a piece of tape and put your case back on.

You can try to edit your build prop to disable the NC Microphone by changing this line - persist.audio.fluence.voicecall=true to false

Are you running stock or on a custom ROM? I also heard that for some the problem went away after flashing a custom ROM.
