It's called WidgetLocker Lockscreen.
No need for rooting though. :]

can you tell me what size widget you used for this? whenever i try this with desktop visualizer, they always end up in the middle. i want them all to align at the left side.

I use the small widget size and resize them with Launcher Pro so they stay on the left side

Nah man, that's from the MIUI ROM which has that as its lockscreen where if you swipe down on the left it goes straight to the phone, middle to unlock, and right to go to text messages

This is pretty awesome. Good job on the background. I know it's a pain to figure out how to space things on the wallpaper so it steps correctly. It looks really cool- and still useful and practical.
