I got it from the Samsung site; just copied the image, cleaned it up, and dropped my screenshot on it.

I think you're asking about the Bluetooth indicator! The screenshot is showing a connected BT device. The icon looks slightly simpler when BT is on but not connected (which sounds like what you see on your phone).

You should turn off BT if you're not using it.

The clock is One More Clock widget...Dyma Dynamics with the weather removed.
Weather is Beautiful weather and the circle battery widget. Matched all the colors.

The wallpaper is just a google image search(Halloween Movie) and darkened it a bit.
The app drawer pumpkin I also google image searched and picked one I liked.
